IV therapy has exploded in popularity. Clinics promise everything from more energy to better skin to faster recovery. But what does the evidence actually say? And when is an IV the right choice over an oral supplement?
Iron Infusion: The Strongest Evidence
Of all IV therapies, iron infusion has the strongest research backing. For patients with low ferritin (stored iron), oral iron supplements are often poorly absorbed and cause gastrointestinal side effects. IV iron bypasses the gut entirely and restores iron stores quickly.
A landmark randomized, double-blind, placebo-controlled trial published in Blood studied 90 premenopausal women with fatigue and ferritin at or below 50 ng/mL who were not anemic. The results were striking. In women with ferritin at or below 15 ng/mL, IV iron reduced fatigue by 1.8 points on a 10-point scale compared to 0.4 for placebo. 82% of iron-treated patients reported improved fatigue compared to 47% on placebo. A separate randomized controlled trial published in CMAJ found that oral iron supplementation also improved fatigue in nonanemic women with ferritin below 50 ug/L, confirming that low ferritin is a real and treatable cause of fatigue, even when hemoglobin is normal. Read the study here.

Nutrient Infusions: Myers' Cocktail and Beyond
Vitamin and mineral infusions (like the Myers' Cocktail, which includes B vitamins, vitamin C, magnesium, and calcium) are popular for general wellness, fatigue, and recovery. The evidence here is more mixed. Some patients report significant improvement in energy and recovery, particularly when they have absorption issues or deficiencies that oral supplements have not corrected.
The rationale is sound: IV delivery achieves blood concentrations that oral supplements cannot. For patients with gut issues that impair absorption, this can make a real difference. But IV nutrient therapy is not a substitute for finding and fixing the root cause. It is a tool, not a solution.
What IV Therapy Cannot Do
IV therapy cannot fix a hormone imbalance. It cannot reverse insulin resistance. It cannot replace the need for proper testing and a personalized protocol. If you are getting IV drips every week but still feel terrible, something deeper is going on.
