    7 Things Your Primary Care Doctor Isn't Testing

    You had your annual physical. Your doctor ordered a basic panel. The results came back "within normal limits." So why do you still feel exhausted, foggy, and not like yourself?

    The answer is simple. The standard annual physical is designed to catch disease, not to optimize health. It checks whether you are sick, not whether you are thriving. That gap leaves a lot of room for silent problems to hide.

    1. Vitamin D (25-Hydroxyvitamin D)

    Most doctors check vitamin D only if you ask. Even when they do, they often consider 30 ng/mL "sufficient." But research shows that optimal levels for bone density, immune function, and fall prevention sit higher, around 40 to 60 ng/mL. A review published in Metabolites found that the majority of disease-specific recommendations set a lower limit of 75 nmol/L (about 30 ng/mL) and an upper target of about 125 nmol/L for optimal health outcomes.     2. Fasting Insulin

    Fasting glucose and A1c are standard, but fasting insulin is rarely checked. That is a problem. Insulin can be elevated for years before glucose ever moves. High fasting insulin is one of the earliest signs of insulin resistance, the metabolic driver behind stubborn weight, fatigue, and eventual type 2 diabetes. A review in Advances in Medical Education and Practice identified insulin resistance as the most important predictor of metabolic dysfunction. Read the study here.

    3. Ferritin (Iron Stores)

    You can have normal hemoglobin and still be iron deficient. Ferritin measures your stored iron, and low ferritin is a major cause of fatigue in women, even when you are not technically anemic. A landmark randomized controlled trial published in Blood found that intravenous iron significantly improved fatigue in nonanemic women with ferritin below 50 ng/mL. Among those with ferritin at or below 15 ng/mL, 82% reported improved fatigue compared to 47% on placebo. Read the study here.

    4. Free Testosterone and Free Estradiol

    Total hormone levels only tell part of the story. What matters is how much hormone is actually available to your tissues. Free testosterone and free estradiol give a far more accurate picture of hormonal balance. Low free testosterone in men is linked to decreased libido, fatigue, and reduced muscle mass. A two-year randomized trial published in The Journal of Sexual Medicine showed that testosterone replacement therapy improved sexual activity, desire, and hypogonadal symptoms sustained over 24 months.     5. Homocysteine

    Homocysteine is an amino acid that, when elevated, signals inflammation and increased cardiovascular risk. It is rarely checked in a standard physical but is a powerful marker for B-vitamin status, methylation, and cardiovascular health.

    6. HbA1c Paired with Fasting Insulin

    HbA1c alone tells you your average blood sugar over three months. But paired with fasting insulin, it reveals the full metabolic picture. If your A1c is "normal" but your fasting insulin is high, your body is working overtime to keep blood sugar in range. That is insulin resistance, and it is the quiet driver behind weight that will not budge.

    7. Comprehensive Thyroid Panel (TSH, Free T3, Free T4, Reverse T3, Antibodies)

    Most physicals check TSH only. That is like checking the temperature of one room to assess the whole house. A full thyroid panel includes Free T3 (the active hormone), Free T4, Reverse T3 (which blocks thyroid function), and thyroid antibodies (which reveal autoimmune thyroid disease). You can have a "normal" TSH and still have a thyroid problem.
